Location: Tucson, Arizona & Surrounding Areas

Key Responsibilities

  • Coordinate and deliver hospice care according to each patient’s individualized plan of care

  • Collaborate with the interdisciplinary team, patients, and families to ensure care needs are met

  • Continuously evaluate and adjust care plans based on patient condition

  • Educate patients and families on the hospice process and healthcare decisions

  • Participate in interdisciplinary group (IDG) meetings and case conferences

  • Conduct evaluations for hospice admission and provide on-call coverage, including holidays

  • Supervise and evaluate Hospice Aides and Licensed Practical Nurses (LPNs)

  • Perform death visits in accordance with agency policy

  • Provide dietary counseling and address symptom management needs

  • Maintain accurate and timely documentation, including visit schedules and care plans

Qualifications

  • Graduate of an accredited School of Nursing

  • Current Registered Nurse (RN) license in the State of Arizona, in good standing

  • Hospice experience preferred

  • Strong clinical assessment and problem-solving skills

  • Proficient in time management and case management principles

  • Valid driver’s license and insured, reliable vehicle

  • Effective verbal and written communication skills

  • Ability to administer medications, perform nursing procedures, and manage end-of-life care in accordance with Arizona nursing practice standards
