What You'll Do
Provide skilled hospice nursing care in patients' homes
Assess patient needs and manage pain and symptoms with compassion and clinical expertise
Develop and implement individualized plans of care in collaboration with the interdisciplinary team
Educate and support patients, families, and caregivers throughout the hospice journey
Maintain accurate, timely clinical documentation
Coordinate care with physicians, therapists, social workers, chaplains, and other members of the hospice team
Participate in a shared on-call rotation to ensure continuous, high-quality patient care
What You Bring
Current Arizona Registered Nurse (RN) license
Minimum one year of nursing experience (hospice, home health, or palliative care preferred)
Associate's or Bachelor's degree in Nursing
Current CPR certification
Valid Arizona driver's license, reliable transportation, and current auto insurance
Excellent clinical assessment and communication skills
A compassionate, patient-centered approach to care
Ability to work independently while collaborating effectively with an interdisciplinary team
