What You’ll Do

Admissions Support

• Perform comprehensive assessments to determine hospice eligibility

• Coordinate with providers and caregivers to ensure seamless transitions

• Clearly communicate hospice services and benefits to patients and families

Crisis Intervention & Comfort Care

• Respond to urgent needs with rapid, skilled care

• Manage pain, symptoms, and emotional distress with clinical precision and compassion

• Educate families on what to expect and how to provide safe, comforting care at home

Documentation & Collaboration

• Maintain accurate, timely records in line with regulatory standards

• Contribute to interdisciplinary team meetings and evolving care plans

• Stay current with training and best practices in hospice care

High-Risk Case Management

• Admit and support patients in their final days with unwavering presence and dignity

• Ensure continuity of care, communication, and comfort throughout the journey

What You Bring

• Active Registered Nurse (RN) license in Arizona

• 2+ years of nursing experience (hospice, palliative, or home health strongly preferred)

• Clinical confidence in symptom management and end-of-life protocols

• Exceptional communication and interpersonal skills

• Strong documentation and organizational abilities

• Proficiency in EMR systems and tech-savvy care delivery

• Prior experience with in-home care or community-based nursing
