Hospice Admissions Nurse

Overview:
As a Hospice Admissions Nurse, you will be responsible for facilitating the transition of patients into the hospice care program. You will work closely with patients, their families, and healthcare teams to coordinate and ensure a smooth admission process. This role requires empathetic communication and strong clinical knowledge to assess patient needs accurately and make appropriate care recommendations. This is a full time position.

Responsibilities:
- Conduct assessments and evaluations to determine patients' eligibility for hospice care through comprehensive physical, emotional, and psychosocial assessments
- Collaborate with physicians, caregivers, and other healthcare professionals to gather necessary information for admission into the hospice program
- Provide accurate and compassionate explanations of hospice services, benefits, and obligations to patients and families
- Coordinate communication and paperwork between patients, medical providers, and the hospice team to ensure timely admission
- Develop and implement care plans, considering patients' unique needs and preferences to promote comfort, dignity, and quality of life
- Administer medications, perform procedures, and provide symptom management as necessary to alleviate pain and symptoms
- Provide emotional support and counseling to patients and their families to address their concerns and fears
- Educate patients and caregivers on end-of-life care, including pain management, medication administration, and equipment usage
- Maintain detailed and accurate documentation of all patient interactions, assessments, and care plans according to hospice regulations and policies
- Participate in the coordination of interdisciplinary team meetings to evaluate and update care plans as needed
- Actively participate in ongoing professional development and training to stay updated on hospice care practices, regulations, and advancements

Requirements:
- Current unrestricted Registered Nurse (RN) license in the state of Arizona
- Minimum of 2 years of experience in nursing, preferably in hospice, home health, or a similar setting
- Proven ability to assess, evaluate, and manage patients' physical, emotional, and spiritual needs
- Excellent communication and interpersonal skills to effectively collaborate with patients, families, and interdisciplinary teams
- Strong organizational and documentation skills with attention to detail
- Ability to provide compassionate bedside care and emotionally support patients and families during end-of-life journeys
- Knowledge of hospice principles, regulations, and best practices
- Proficiency in using various computer software and electronic medical records systems

Preferred Qualifications:
- Experience in conducting home visits or community-based nursing
