We are currently seeking a dedicated Hospice Admissions Nurse to join our compassionate care team.
Position Overview:
The Hospice Admissions Nurse plays a vital role in facilitating a seamless transition into hospice care. This position involves conducting comprehensive assessments, providing education to patients and families, and coordinating with healthcare providers to ensure timely and appropriate admissions. The ideal candidate is a skilled clinician with excellent communication skills and a strong commitment to empathetic, patient-focused care.
Key Responsibilities:
Conduct in-depth clinical assessments to determine hospice eligibility, addressing physical, emotional, and psychosocial needs
Collaborate with referring physicians, caregivers, and healthcare providers to obtain and verify relevant medical information
Educate patients and families on hospice philosophy, services, and benefits
Coordinate documentation, referrals, and admissions process to ensure timely and efficient onboarding into hospice care
Develop individualized care plans based on patients' conditions and preferences
Administer medications and provide symptom relief and comfort-focused care as needed
Provide emotional and psychosocial support to patients and their families during the admission process
Educate caregivers on end-of-life care, pain and symptom management, and equipment use
Maintain detailed, timely, and accurate documentation in compliance with hospice policies and regulatory standards
Participate in interdisciplinary team meetings and collaborate on care planning and updates
Stay informed on the latest hospice care practices, guidelines, and compliance requirements through professional development
Required Qualifications:
Current, unrestricted Registered Nurse (RN) license in the State of Arizona
Minimum of 2 years of clinical nursing experience (hospice, home health, or palliative care experience strongly preferred)
Strong clinical assessment skills and the ability to manage complex medical situations
Excellent communication and interpersonal skills
Empathetic approach to patient and family care
Strong attention to detail and organizational skills
Proficient in electronic medical records (EMR) systems and common office software
Preferred Qualifications:
Experience in community-based or in-home nursing
Familiarity with hospice eligibility criteria and CMS guidelines
Additional Information:
This is a full-time, Monday through Friday, 8:00 AM – 5:00 PM position. Occasional travel to patient homes may be required.
