Registered Nurse (RN) Case Manager – Full-Time Hospice

Dependable Hospice is seeking an experienced and compassionate RN Case Manager to join our growing Phoenix Hospice team.

This is an excellent opportunity for a dedicated nurse who enjoys building meaningful relationships with patients and families while providing exceptional end-of-life care.

As a Hospice RN Case Manager, you'll coordinate individualized care plans, manage patient needs, and collaborate with an interdisciplinary team to ensure comfort, dignity, and quality of life.

What You'll Do

• Manage and coordinate care for an assigned caseload of hospice patients

• Perform comprehensive nursing assessments and ongoing evaluations

• Develop, implement, and update individualized plans of care

• Provide symptom management, pain control, and comfort-focused nursing care

• Educate and support patients, families, and caregivers

• Collaborate with physicians, hospice aides, social workers, chaplains, and other team members

• Ensure accurate, timely clinical documentation and regulatory compliance

• Participate in interdisciplinary team meetings and care coordination

What We're Looking For

• Current, unrestricted Arizona RN license

• Hospice nursing experience preferred

• Strong clinical assessment and symptom-management skills

• Knowledge of hospice philosophy and end-of-life care

• Excellent communication, organization, and time-management skills

• Valid driver's license and reliable transportation

• Compassionate, independent, and committed to patient-centered care
